Website Use
The ProvCreda website provides general information about provider credentialing, payer enrollment, and related healthcare administration services. Website content is not legal, billing, compliance, or medical advice.
Portal Access
Portal access is restricted to approved users. Users are responsible for keeping login credentials secure and using only authorized access. Provider users may access only records associated with their approved organization. Workforce and administrative access for PHI workflows may require MFA, role-based permissions, session timeout controls, audit logging, and access review.
No Public Submission of Sensitive Records
Public forms should not be used to submit passwords, PHI, payer portal credentials, Social Security numbers, date-of-birth details, license images, claims information, or sensitive provider records. Secure intake channels should be established separately where required.
HIPAA and Regulated Information
Any work requiring ProvCreda to create, receive, maintain, or transmit PHI or ePHI must be governed by the appropriate written agreement, secure workflow, and production safeguards. The public website and public contact form are not intended for regulated information intake. Approved PHI workflows are expected to use secure portal or encrypted channels, BAA-covered vendors where required, minimum necessary handling, audit controls, restricted storage, and documented administrative, physical, and technical safeguards.
Service Engagements
Specific service scope, fees, responsibilities, confidentiality obligations, and compliance requirements should be documented in a separate written agreement between ProvCreda and the healthcare organization.
